LPN Coordinator
Apply Job ID LPNCO012501 Date posted 12/03/2025Trustbridge Hospice, a part of Empath Health is seeking a caring and motivated LPN Coordinator to work with us in our West Palm Beach location to provide coordination of our staff visits on the following schedule: every Friday/Saturday/Sunday 8:00am – 8:30pm.

What you'll Do
- The LPN Coordinator is responsible for coordinating with Managers and team members in patient/family direct and indirect care needs and provides quality professional intervention based on protocols and policy regarding the physical, emotional and psychological needs.
- Interacts with management, physicians and field staff utilizing a teamwork approach.
- Serves as a professional leader in assessing, planning, guiding and evaluating patient care activity.
- Pledge to make the comfort and care of patients and families the highest priority.
- Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
- Ensure the complete satisfaction of the patient and family.
What You'll Need
- Current license to practice as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing.
- Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs.
- Basic computer skills and EMR software experience a plus.
