Director Clinical Optimization (TB)

Position Qualifications / Requirements:
- Bachelor’s degree in nursing (master’s preferred).
- Active RN or APRN license
- 3–5 years of experience in healthcare operations, hospice, or home health preferred.
- Knowledge of hospice regulations (Medicare/Medicaid, CMS Conditions of Participation, state regulations).
- Experience with QAPI programs and process improvement methodologies (Lean, Six Sigma a plus).
- Strong analytical, organizational, and communication skills.
- Proficiency in EMR systems (Home Care Home Base – strongly preferred), data reporting tools, and Microsoft Office Suite.

Job Duties / Responsibilities:
Operational Improvement & Process Optimization
- Evaluate current workflows, policies, and procedures to identify inefficiencies and gaps.
- Develop and implement process improvement initiatives aligned with hospice best known practices and compliance standards.
- Monitor performance metrics (clinical, financial, and operational) to track progress and identify trends.
- Collaborate with interdisciplinary teams to standardize practices across sites and branches.
Quality & Compliance
- Support quality assurance and performance improvement (QAPI) initiatives.
- Ensure compliance with CMS Conditions of Participation, state regulations, and hospice-specific standards of care.
- Partner with Quality and Compliance teams to monitor clinical outcomes and patient satisfaction, driving continuous improvement.
Data Analysis & Reporting
- Collect, analyze, and interpret operational and clinical data to inform decision-making.
- Create and present reports and dashboards for leadership on key performance indicators (KPIs), including census growth, length of stay, productivity, and quality measures.
- Recommend strategies to improve care delivery, financial performance, and operational efficiency.
Training & Support
- Provide education and guidance to leaders on new processes, policies, and best practices.
- Act as a resource for operational problem-solving and process standardization.
- Support leaders in implementing new initiatives and ensuring adoption within their teams.
Technology & EMR Optimization
- Serve as a subject matter expert for clinical documentation, workflow, and reporting within the EMR, with a focus on Home Care Home Base (HCHB).
- Partner with IT and Clinical Informatics teams to enhance EMR utilization, improve user adoption, and ensure documentation supports compliance and operational goals.
- Identify opportunities to leverage technology and automation to streamline processes and improve efficiency.
Leadership & Collaboration
- Act as a strategic partner to senior leadership, contributing to organizational planning and decision-making.
- Lead cross-functional initiatives that support clinical excellence, staff efficiency, and improved patient care.
- Build strong relationships with frontline clinicians, fostering a culture of continuous feedback and improvement.
