Clinical Social Worker MSW - Float (Palm Beach area)

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and patient-focused Clinical Social Worker MSW Float to join our team. This position works our Palm Beach territory (West Palm Beach/Loxahatchee/Wellington/Juno/Belle Glade).

This full-time position works 8:30 a.m. – 5 p.m and every other weekend. In this important role, you will provide essential psychosocial assessments and interventions that help patients and families navigate end-of-life care—directly enhancing the quality, dignity, and comfort of their hospice experience.

Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

What You’ll Do

  • Provide psychosocial assessment and intervention for patients and families within his or her scope of practice as a member of the interdisciplinary team
  • Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family.
  • May supervise interns and unlicensed social workers

What You’ll Need

  • Graduate with a Master’s degree in Social Work from an approved college or university accredited by the Council on Social Work Education.
  • One year of experience as a MSW or hospice experience preferred.
  • Ability to work effectively and flexibly as part of a collaborative interdisciplinary team.
  • Strong written and oral communication skills.
