Empath Health is a nonprofit integrated network of care that provides expert, life-changing health care to those with chronic and advanced illnesses. Empath Health includes hospice, palliative home health and physician services, independent living assistance and day care for seniors, HIV services and advance care planning.

EPIC (Empath Partners in Care) is a nonprofit provider of medical case management, counseling and pharmacy services to Tampa Bay area residents with HIV. As a member of Empath Health, our focus is helping clients achieve treatment success so they can live a healthy life, providing support to caregivers and family members and educating the community on the prevention of HIV.

We are currently seeking an experienced and detail-oriented Supportive Services Specialist to join our team. The Supportive Services Specialist will provide HOPWA related services. This includes facilitation of support groups, client intakes and assistance with nutritional and transportation programs.

Other essential functions and responsibilities of this position include:

• Conduct new client intakes and needs assessments and make referrals to internal and external programs as appropriate.
• Assist clients in becoming involved in the counseling process so that they may resolve problems that hinder their ability to maintain stable housing.
• Provide experienced, professional counseling; assist and support clients in developing and/or maintaining a responsible and functional lifestyle
• Recognize problems beyond the counselor’s training, skill or competence, and be willing and able to utilize other appropriate professional services
• Participate in case consultation and client staffing
• Follow all HIPAA regulations and requirements
• Maintain case records, progress notes, group notes, and all other progress reports, court updates, and documentation as required.
• Assist clients in setting treatment planning goals, monitor progress toward these goals, and participate in treatment planning review.
• Work with and communicate with peers, supervisors and clients
• Link clients to available substance abuse programs and community resources
• Work cooperatively with community agencies to ensure the availability of substance abuse treatment services for clients
• Provide nutritional assistance to clients in need via meal preparation and food pantry.
• Perform other duties as assigned

• Education and/or Experience: Bachelor’s degree (B.A.) from four-year college or university in Addictions or related field; or one to two years related experience and/or training; or equivalent combination of education and experience.
• Excellent clinical, organizational, communication, and writing skills required
• Computer proficiency required
• Two years’ experience in counseling preferred but not required
• Valid Florida’s Driver’s License
• Therapy & Counseling: Knowledge of principles and theories of addiction, the 12 Steps, methods and procedures of diagnosis, treatment and rehabilitation of addiction disorders, addiction treatment modalities, and relapse prevention methods
• Knowledge of HIV/AIDS, issues surrounding diagnosis and medications and treatments.
• Active Listening – Giving full attention to what other people are saying, taking time to understand points being made, asking questions as appropriate, and not interrupting at inappropriate times
• Critical Thinking – Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions and approaches to problems
• Group Counseling – Effectively use different group techniques in order to enhance the knowledge and behavior of clients
• Service Orientation – Actively looking for ways to serve EPIC clients
• Learning Strategies – Selecting and using training / instructional methods and procedures appropriate for the situation when learning or teaching new things
• Time Management – Manage one’s schedule so that client appointment times are consistent and that paperwork can be managed within working hours
• Assessment – Assess client level of need and work effectively within that level of need
