Empath Home Health, a member of Empath Health, is seeking a compassionate Home Health RN to provide skilled nursing care for patients in their homes throughout Hillsborough County (Tampa, Lutz, Odessa, Carrollwood, and surrounding areas). In this full-time role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.
Schedule: Monday–Friday, with required weekend/holiday on-call rotations.
Locations: home health visits in Hillsborough County (Tampa, Lutz, Odessa, Carrollwood, and surrounding areas)

What You’ll Do
• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
Position Requirements
• Graduate of an approved nursing program with an active Florida RN license
• BSN or MSN preferred
• Minimum of 1 year of direct patient care experience; prior home health RN experience preferred
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
