Pharmacy Care Coordinator

The Pharmacy Care Coordinator is a member of the Pharmacy team, reporting to the Director of Pharmacy. This position, under the guidance of a Licensed Pharmacist, will coordinate 340B pharmacy services to enrolled clients by working with both the pharmacy and EPIC to ensure continuity of care. This position will also work to ensure compliance with all aspects of the 340B pricing program, assist with reporting out all 340B activity to stakeholders, and maintain the 340B inventory replenishment.

JOB DUTIES/RESPONSIBILITIES:

  1. Assists the Pharmacist with the daily operations of the pharmacy care programs.
  2. Answers incoming calls to the pharmacy and handles caller with pleasant and friendly demeanor. Handles or triages the call appropriately. Manages and handles all incoming faxes appropriately and efficiently.
  3. Completes data entry of patient and prescription information into pharmacy system.
  4. Completes patient welcome and refill compliance calls according to talking points and procedures. Ability to describe the program to patients and prescribers.
  5. Manages communication with prescribers to resolve issues and to provide updates of patients and prescription statues.
  6. Adjudicates prescription claims in the appropriate pharmacy system to ensure accurate billing and payment for product.
  7. Completes benefits investigations for patients and resolve any payer rejections, such as prior authorization coordination, and appeals.
  8. Enrolls and coordinates patient financial assistance and support services, like copay cards, funding assistance and manufacturer support programs.
  9. Works with patients to synchronize medication dispenses and assist with adherence programs.
  10. Coordinates prescriptions for pick up or home delivery. Tracks all deliveries and ensures signature paperwork is filed appropriately.
  11. Maintains compliance with company policies and procedures, and complies with all laws regulating pharmacy services. Maintains all PHI according to HIPAA rules.
  12. Prepares reports by collecting and summarizing information including dispensing records, 340B compliance reports, delivery records, and inventory records.
  13. Works with Operations Manager/Supervisor to maintain compliance with the 340B pricing program.
  14. Collaborates with 340B Covered entity on the enrollment of new clients to ensure smooth transitions from previous pharmacies.
  15. Adhere to driver safety guidelines.
  16. Performs other duties as assigned by the Pharmacist.

POSITION QUALIFICATIONS/REQUIREMENTS:

  • Education and/or Experience: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Licensure: Florida Pharmacy Technician License
  • Continuing education: 20 Hours Biennial
  • Knowledge of Pharmacy Benefit Management (PBM) procedures and processes.
  • Demonstrate ability to compile and analyze data from various sources.
  • The ability to work with minimal supervision, prioritize and multi-task with little or no disruption of work flow; strong analytical and problem solving skills.
  • Intermediate Excel and report skills, with additional experience entering and retrieving data on a computer.
  • Excellent communication and organization skills; composing written statements and proof-reading correspondence.
  • Good knowledge of: medical and pharmaceutical terms and abbreviations.
  • Skill in typing/keyboarding at a minimum of 30 words per minute.
  • Minimum 3-5 years of experience in Retail, Managed care, PBM, Hospital, Home Health Care, or Long Term Care Pharmacy settings desired.
  • Demonstrate an understanding of 340B program requirements.
