Social Work Coordinator

PACE, a member of Empath Health is currently seeking a Social Work Coordinator to join our team.

Under the supervision of the Executive Director, Center Manger, and/or Social Work Manager, the Social Work Coordinator coordinates the Social Work practices and services for PACE participants and families for the assigned PACE Program(s).

Responsibilities include but are not limited to: Training and Orientation; Task Supervision of Social Work staff; Assignment of caseloads, regulatory assessments, and other Social Work tasks; Chart Audits; Psychosocial Assessments, Psychoeducation, and Counseling of participant, caregiver, or other participant representatives. Social Work interventions include individual assessment and intervention, coordination of care with community partners, participant and family education, assessment, counseling, case management, advocacy for participant and caregivers, and disenrollment procedures. 

The Social Work Coordinator ensures the proper and timely completion of Social Work assessments per state and federal regulations, training and support of both new and experienced social workers as well as facilitation of ongoing social education.  The Social Work Coordinator carries a reduced caseload and provides both Consultation with complex cases as needed.  The Social Worker Coordinator is the liaison between the Social Work Department and PACE Leadership, caregiver representatives, and community agencies.

Position Requirements:

  • Education and/or Experience: Master of Social Work (MSW) degree from a school of Social Work accredited by the Council on Social Work Education
  • At least one year of social work experience in a healthcare setting on a multidisciplinary team is preferable (the one-year Masters level internship would meet this requirement) and has a current Florida license,
  • Skills and Knowledge:
      • Licensed Clinical Social Worker (LCSW) in the State of FL preferred
      • 1-3 years of Supervisory experience preferred.
      • At least one year’s experience working with the frail or elderly population.
      • Ability to provide psychosocial assessment and individual, family and group counseling.
      • Effective verbal/written communication skills with the ability to maintain accurate records and to prepare clear and concise reports, correspondence, and other written materials.
      • Training and/or mentoring experience and ability to complete performance objectives, measures, and evaluations.
      • Good public speaking skills with all size groups.
      • Ability to communicate clearly and effectively verbally and written.
      • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
      • For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
