Empath Health is seeking a compassionate Registered Nurse (RN) to join our interdisciplinary PACE team in Pinellas Park, FL. In this role, you’ll provide personalized nursing care that helps older adults remain independent and live with dignity—delivering services across homes, facilities, and our Day Center. What You’ll Do

  • Provide skilled nursing care to PACE participants across home, facility, and Day Center settings
  • Perform comprehensive assessments and create individualized care plans
  • Monitor patient progress, adjust care plans, and document outcomes
  • Collaborate with the interdisciplinary care team to ensure seamless, holistic care
  • Educate participants and caregivers on health management and preventive care

Position Requirements

  • Active Florida RN license (or eligibility)
  • Minimum of 1 year of nursing experience with geriatric or chronically ill populations
  • CPR certification (AHA or Red Cross)
  • Strong clinical assessment, communication, and care coordination skills
  • Previous PACE, home health, or long-term care experience preferred, but not required
