PACE Health Information Technician PRN

Suncoast PACE, a member of Empath Health, is seeking a Health Information Technician to join the Empath team.

The Health Information Tech is responsible for maintaining the integrity of the electronic health record so that it is available to those who require and are authorized to access it, in accordance with policies and procedures of Empath Health/Suncoast PACE.

The Health Information Technician:

  • Processes information in an accurate & timely manner according to established procedure.
    1. New enrollments (includes verifying demographics, assuring required documentation is present, signed and dated)
    2. Deaths
    3. Disenrollments
    4. Transfers to residence, hospital, or nursing home.
  • Tracks “Green Folder” for pre-enrollment and ensures collection of all required items prior to PACE enrollment. Alerts the PACE center manager/designee of missing documents.
  • Completes Enrollment checklist/audit.
  • Scans the participant chart in Agency-approved format.
  • Assures that time-sensitive documents are processed the day of receipt of the document.
  • Ensures physician documents are signed and dated appropriately before scanning.
  • Assures that follow-up on unsigned documents is timely and consistent; enters comments into the medical record for physician contacts and updates physician order data. Notifies the Center Manager/designee of any problems in obtaining signed documents and orders after two unsuccessful attempts.
  • Maintain assessment tracking form/track all required assessments for completion to ensure regulatory compliance.
  • Notifies IDT of POC meetings according to team requirements and schedules. Collaborates with IDT on a regular basis.
  • Processes incoming mail on the day received. Assures that time-sensitive documents are logged and scanned expeditiously.
  • Collaborates with the Empath Health Director of HIM as necessary to clarify and verify information.
  • Respects the confidentiality of information related to patients, volunteers, visitors and co-workers.
  • Continuously strives to contain costs through the proper use of supplies.
  • Participates in PACE meetings, HIT meetings, Agency required in service programs, and department QI.

Requirements:

  • High School diploma or equivalent required
  • Three years experience in Medical Records/Health Information preferred
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact
  • Must have a valid driver’s license, proof of insurance, and have means of transportation
  • Knowledge of Suncoast PACE Policies & Procedures
  • Strong clerical and computer skills
  • Experience working with Outlook and Excel; experience in electronic health record systems preferred
  • Excellent organizational skills
  • Accurate and detail oriented
  • Able to work in close proximity to others in a team environment
