Social Work Supervisor- PACE

Under the supervision of the Operations Manager and/or Executive Director, the Social Work Supervisor plans and supervises the organization and implementation of social services to PACE participants and families. Responsibilities include but are not limited to: Supervision of social work staff, appropriate assignment of social workers to caseloads based on territory, skills and equity, assessment, treatment, teaching and counseling of participant, caregiver, or other appropriate representatives. The Social Work Supervisor ensures the proper and timely completion of social work assessments per state and federal regulations, training, and support of new social workers as well as facilitation of ongoing social work support and education. The Social work Supervisor does not carry a caseload but assists with complex cases as needed. Social Work interventions should include individual participant contacts; appropriate collateral contacts; participant and family education, assessment, and counseling; provision of resources; ongoing case management; advocacy to ensure participant and caregiver needs are met and addressed; and disenrollment procedures. The Social Worker Supervisor is the liaison between the Social Work Team and PACE Leadership, caregiver representatives, and community agencies. Directly reports to the PACE Operations Manager.

• Must have a valid driver’s license, proof of insurance and have means of transportation.
• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
• Successfully complete a Competency Assessment upon hire and receive annual competency assessments.
• Skills and Knowledge:
o A Licensed Clinical Social Worker in the State of FL
o 2 years supervisory experience preferred.
o At least one year’s experience working with the frail and elderly population.
o Ability to provide psychosocial assessment and individual, family and group counseling.
o Effective verbal/written communication skills with the ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials.
o Training and/or mentoring experience and ability to complete performance objectives, measures and evaluations.
o Good public speaking skills with all size groups
o Ability to communicate clearly and effectively verbally and written.
