Regional Access Director

The Regional Access Director is a qualified health and human service professional who is responsible for facilitating admissions for all Empath Health service lines.  This leader ensures access to the care network of Empath Health and facilitates an optimum person-centered experience seven days/week, while clearly exhibiting passion, excellence and a desire to serve. Responsibilities include leading, developing and monitoring the development and performance of the Admissions department interdisciplinary team members.  In addition, this leader will have responsibility for ongoing development and implementation of best practices within Admissions with a focus on service to the patients and families we serve and overall results.

JOB DUTIES/RESPONSIBILITIES:

Admissions Department

  1. Access all service opportunities on an ongoing basis, consistent with Empath Health’s vision of Full Life Care for members of the communities in which we serve.
  2. Lead and develop a high performing team through colleague engagement, feedback, performance evaluation, and coaching.
  3. Drives service and overall results through team collaboration and ensuring accountability of each individual member of the team.
  4. Identifies and manages strategic customer service, access and admission alliances to position Empath Health for the future of the ever-changing health care industry.
  5. Serve as lead contact for access escalation 24/7.
  6. Lead department from initial call to successful admission with proven average annual statistics.
  7. Leads and represents the Admissions department in decision-making while creating all related Policies/Procedures to ensure quality, integrity, compliance, customer focus at all times.
  8. Initiate, develop, implement, evaluate, staff’s areas of responsibility to ensure rapid response while ensuring optimum compliance to regulatory standards, always keeping the patient/family needs at the forefront.
  9. Exhibit ability to think creatively in recommending proper follow through and service excellence to provide the right care at the right time by working directly with internal and external colleagues.

Leadership

  1. Supports and demonstrates the Empath Mission, Vision and Values on a consistent basis.
  2. Understands and supports all service lines across Empath through collaboration and demonstrates willingness to share knowledge, resources, and talent to further advance Full Life Care to individuals across all of our service areas.
  3. Work directly with Chief Medical Officer and Medical Directors to ensure care program access at all times.
  4. Partner with Business Development and Community Outreach to ensure excellent relationships with community health partners.
  5. Serve as a resource to all direct reports, as well as inter-departmental staff to insure continual access, compliance, customer service, collaborative efforts seven (7) days/week.
  6. Ensure data accuracy, transparency and understanding.
  7. Lead, mentor and coach Admissions staff members across all disciplines, including, but not limited to managers, navigators, admission nurses, social workers and patient concierges to ensure access to the care network of Empath Health within regulatory guidelines.
  8. Represent the agency in providing education to the community regarding the care network offered within Empath Health.
  9. Work directly with department leads and directors to ensure ongoing internal relationship-building and collaborative efforts to insure an excellent person-centered experience.
  10. Represent Admissions at director-level meetings, task forces, PIPs, committees, special assignments, decision-making processes to maintain the highest standards of quality of care.
  11. Lead department by continual positive example-setting, mentoring, role modeling, always working to insure an excellent patient experience with a calming sanctuary in conjunction with a sound business.
  12. Work directly with Health Information Management Director to always ensure integrity of electronic medical record.
  13. Work directly with Vice President of Organizational Excellence & Compliance, Clinical Practice Director, and Director of Social Work to ensure optimum integrity and best practice beginning at time of admission to Empath Health.

Stewardship

  1. Develop, implement, and monitor annual budget in collaboration with Finance Department and Vice President for Strategy and Business Development.
  2. Monitor monthly financial statements to ensure budgetary compliance.
  3. Work directly with Director of Reimbursement to insure proper financial classifications of new admissions within compliance guidelines.

Planning

  1. Align the department’s daily work with the strategic direction of the agency as a whole.
  2. Lead department planning for annual goals contributing to the Strategic Plan of the agency.
  3. Actively participate in agency planning efforts to exceed the needs of the community.
  4. Monitor census trending daily through collaborative efforts with intra-agency leaders.
  5. Champions the care experience and engages staff to continuously enhance the care experience by inspiring a culture of person-centered care and serving as a role model for care experience excellence.
  6. Identifies key performance measures and analyzes trends. Implements strategic planning for process improvement and unencumbered access to Empath services.
  7. The Regional Care Navigation Director will perform other duties as specified.

POSITION QUALIFICATIONS/REQUIREMENTS:

  • Education and/or Experience: Bachelor’s degree (B.A.) from four-year college or university required. Master’s degree preferred.
  • Three years Hospice experience with at least three years of management experience preferred.
  • Seven to ten total years of experience as an RN preferred.
  • Licensure: RN
  • Continuing education: Must acquire continuing education and training related to regulatory and professional practices.  Demonstrates advancement in knowledge and expertise.                                                                                                                                                                                                                                                                                                                                                                                                                                                                     
  • Certifications: Valid CPR certification from the American Red Cross or American Heart Association; Current certifications as applicable to their professional discipline.
  • Colleague must sign a Business Protection Agreement.
