Hospice of Marion County, a part of Empath Health is seeking a Care Navigator to join our team as a key member of the Palliative Care team by providing administrative support to the team, patients, families, and care partners.  Maintains excellence in customer service by being caring, compassionate, empathetic and responsive to the needs of all who reach out to us internally and externally.

What you'll Do

  • In collaboration with Care Navigation, acts as the central point of contact for the Palliative Care Consultation Service and assures timely response to the referral source and patient/patient representative
  • Responsible for the timely scheduling of initial and follow-up palliative care consultation visits by physicians, ARNPs, and any other team members in all settings
  • Assists in obtaining and verifying insurance information and coverage.
  • Ensures a consent for services and other initial or required documentation is explained, completed and received from the patient or designated patient representative.  These documents will be entered into the EMR
  • Triage incoming patient, family, and community calls to appropriate team members and assist patients with continuity of care
  • Schedule: Monday-Friday 8:00AM-5:00PM

What You'll Need

  • Education and/or Experience: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Excellent verbal and written communication skills with patients/families, healthcare providers, direct co-workers and peers and Superior customer service skill
  • Basic knowledge of healthcare reimbursement, including coding
  • Ability to exhibit professionalism, maintain respect for confidentiality and ability to work under minimal supervision
  • Minimum of two (2) years of administrative experience in palliative care, hospice, home health or related field
