ESSENTIAL JOB RESPONSIBILITIES:

Include the following.  Other duties may be assigned.

  • Maintains effective communication with the NRDEP and GUIDE colleagues, Pathways, Business Development Department, referral sources and local first responder departments.
  • Carries out responsibilities consistent with the guidelines for Care Navigation within the GUIDE model.
  • Maintains confidentiality of person with dementia/family information in accordance with agency and

professional standards.

  • Facilitates Virtual Dementia Tour® trainings, as needed.
  • Provides dementia care consultations for individuals with dementia and/or their caregivers to assess and uncover any psychosocial needs and makes home visits, as needed.

QUALIFICATION REQUIREMENTS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • A Master’s Degree in Social Work plus minimum of (1) one year experience as a caseworker with an emphasis on counseling, crisis management, community agency referral and grief support, preferred.
  • Must have a Valid Florida drivers License, maintain current auto insurance coverage and have access to a reliable automobile.
  • Experience in developing collaborative relationships with referral sources and first responders.
  • Ability to function independently and to adjust work hours to meet with program participants and to provide outreach and educational programs to family caregivers, healthcare professionals and first responders.
  • Ability to write routine reports and correspondence.
