QUALIFICATIONS
- Graduate of vocational/technical school with current license in respective state.
- Minimum one (1) year experience and/or training in home health care preferred.
- Background check acceptable to the Agency.
- Current CPR Certification
- Intravenous Certification preferred.
- Valid driver’s license and proof of automobile liability insurance.
- Able to work autonomously with minimum supervision.
- Demonstrated organizational skills and detail-oriented.
- Demonstrated computer skills.
- Demonstrated time management skills.
- Demonstrated interpersonal skills.
- Demonstrated verbal and written communication skills.
- Ability to communicate in English.
- Able to work as a team member.
PHYSICAL AND MENTAL REQUIREMENTS
- Must be able to work under stress and in a fast-paced environment.
- Must be able to work under conditions that require sitting, standing, walking, lifting, bending, reaching, pulling, fingering, grasping, talking, hearing and seeing.
- Must be able to travel to patient’s homes and to agency-sponsored programs and meetings.

DUTIES AND RESPONSIBILITIES
- Demonstrate professional conduct and ethics according to agency and procedure.
- Maintain confidentiality of patient information and all Agency operations.
- Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
- Take and record patient’s vital signs.
- Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
- Administer prescribed medications by subcutaneous or intramuscular injection and document appropriately.
- Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
- Prepare equipment and materials for treatment observing aseptic techniques as required.
- Assemble and use medical equipment efficiently.
- Collect samples such as urine, blood and sputum for testing.
- Administer intravenous therapy under the direction of a Registered Nurse, as state law permits.
- Perform patient and family instruction on diet, medication and treatment.
- Record dietary and fluid intake and output of patient.
- Assist patients in ambulation, mobility and therapeutic exercise regimen.
- Assist patients in learning self-care techniques.
- Consult with the Clinical Coordinator and/or Registered Nurse frequently, giving progress reports and updates about the patient.
- Report any change in the patient’s condition to the Clinical Coordinator and/or Registered Nurse and document change on appropriate forms for the clinical record.
- Maintain an effective working relationship with clients, staff, representatives of other departments and community agencies.
- Document, review and demonstrate compliance with the Agency’s policies and procedures and federal, state and local regulations.
- Participate in case conferences.
- Maintain confidentiality of clinical records for each patient receiving care.
- Perform other job duties and responsibilities as assigned by the Director of Nursing including participating in performance improvement activities; i.e., record reviews and performance groups.
