QUALIFICATIONS

1. Graduate of vocational/technical school with current license in respective state.
2. Minimum one (1) year experience and/or training in home health care preferred.
3. Background check acceptable to the Agency.
4. Current CPR Certification
5. Intravenous Certification preferred.
6. Valid driver’s license and proof of automobile liability insurance.
7. Able to work autonomously with minimum supervision.
8. Demonstrated organizational skills and detail-oriented.
9. Demonstrated computer skills.
10. Demonstrated time management skills.
11. Demonstrated interpersonal skills.
12. Demonstrated verbal and written communication skills.
13. Ability to communicate in English.
14. Able to work as a team member.

PHYSICAL AND MENTAL REQUIREMENTS

1. Must be able to work under stress and in a fast-paced environment.
2. Must be able to work under conditions that require sitting, standing, walking, lifting, bending, reaching, pulling, fingering, grasping, talking, hearing and seeing.
3. Must be able to travel to patient’s homes and to agency-sponsored programs and meetings.

DUTIES AND RESPONSIBILITIES

1. Demonstrate professional conduct and ethics according to agency and procedure.
2. Maintain confidentiality of patient information and all Agency operations.
3. Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
4. Take and record patient’s vital signs.
5. Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
6. Administer prescribed medications by subcutaneous or intramuscular injection and document appropriately.
7. Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
8. Prepare equipment and materials for treatment observing aseptic techniques as required.
9. Assemble and use medical equipment efficiently.
10. Collect samples such as urine, blood and sputum for testing.
11. Administer intravenous therapy under the direction of a Registered Nurse, as state law permits.
12. Perform patient and family instruction on diet, medication and treatment.
13. Record dietary and fluid intake and output of patient.
14. Assist patients in ambulation, mobility and therapeutic exercise regimen.
15. Assist patients in learning self-care techniques.
16. Consult with the Clinical Coordinator and/or Registered Nurse frequently, giving progress reports and updates about the patient.
17. Report any change in the patient’s condition to the Clinical Coordinator and/or Registered Nurse and document change on appropriate forms for the clinical record.
18. Maintain an effective working relationship with clients, staff, representatives of other departments and community agencies.
19. Document, review and demonstrate compliance with the Agency’s policies and procedures and federal, state and local regulations.
20. Participate in case conferences.
21. Maintain confidentiality of clinical records for each patient receiving care.
22. Perform other job duties and responsibilities as assigned by the Director of Nursing including participating in performance improvement activities; i.e., record reviews and performance groups.
