Description

9:00-5:00 Mon-Fri with an on-call rotation approx once per month.

DUTIES AND RESPONSIBILITIES
1. Demonstrate professional conduct and ethics according to agency policy and procedure.
2. Maintain confidentiality of patient information and all Agency operations.
3. Provide nursing services in compliance with agency policies and procedures and federal, state and local regulations.
4. Perform initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
5. Initiate the plan of care and revise as necessary.
6. Provide nursing services and/or treatments indicated on plan of care.
7. Counsel and educate the patient and family on disease process, self-care techniques and prevention strategies.
8. Initiate appropriate preventive and rehabilitative nursing procedures.
9. Prepare clinical and progress notes, including the use of automated documentation systems if applicable.
10. Coordinate services given by other members of the health care team.
11. Make referrals to other medical services, as needed.
12. Inform the physician and other staff of changes in the patient’s status and or needs.
13. Evaluate the effectiveness and outcomes of care.
14. Supervise Licensed Practical Nurses/Licensed Vocational Nurses and other paraprofessionals providing services.
15. Assigns patients to Home Health Aides and supervises patient’s personal care.
16. Plan for the patient’s discharge from home health services.
17. Promote patient and family independence from home health care.
18. Maintain effective working relationships with clients, staff, representatives of other departments and community agencies.
19. Participates in patient case conferences.
20. Participates in Nursing and Administration On-call rotation.
21. Perform other job duties and responsibilities as assigned by the Director of Nursing/Clinical Coordinator including participating in performance improvement activities; i.e. record reviews and performance improvement groups.

QUALIFICATIONS
1. Degree in Nursing preferred.
2. Current licensure as a Registered Nurse in the respective state.
3. One (1) year home health care experience preferred.
