Senior Quality Improvement Specialist

Remote position, but must live in SW Florida.  Monday-Friday, daytime hours.  Strong knowledge of OASIS & Home Health Quality measures required.

JOB DUTIES/RESPONSIBILITIES:

1.    Ongoing monitoring and trending of key indicators to drive the success of publicly reported agency quality measures to meet and exceed the state and national standard.

2.    Uphold the organization’s mission, vision, and values to provide full life care.

3.    Drive quality through education and example. Facilitate/ guide task forces groups using best practice improvement models and framework.

4.    Develop and maintain an in-depth knowledge of current regulatory reporting.  Position may focus on specialized area of expertise.  Examples PACE, EPIC, Home Health, Hospice and Palliative Care regulatory quality measures.

5.    Listen to the needs of the customer through service recovery, surveys, and interactions. Utilize information to improve care experience.

6.    Utilizes systematic process for data collection, analysis, trending and reporting which is consistent with agency needs, accreditation standards, and regulatory requirements. Prepare and present findings through reports, graphs, and power point applications.

7.    Maintain and ensure confidentiality, security, and integrity of protected healthcare information in accordance with agency and regulatory requirements.

8.    Demonstrates awareness of self as a role model for quality and service excellence.  Facilitator of change functioning as an effective high performing team member.

9.    Embrace collaboration as a team member with the ability and strength to facilitate diverse meetings.

10.  Demonstrates resilience, perseverance, and adaptability, specifically during moments of high volume & high stress.  Flexibility adapting to changes in priorities and direction.

11.  Performs other duties as required by the Vice President of Quality, Manager of Quality and Service Excellence based on division priorities.

12.  25% travel expected - Ability to travel to branch locations.

POSITION QUALIFICATIONS/REQUIREMENTS:
• Education and/or Experience: Registered Nurse or alternative licensed healthcare professional preferred with a minimum of an associate or bachelor’s degree in related field required. Master’s degree strongly preferred. (A combination of work experience and education may be considered as meeting the requirement)
• CPHQ preferred/ or obtain within one year of employment.
• COS-C preferred /or obtain within 6 months of employment.
• Three to five years of direct skilled home health care management experience required in clinical, quality, regulatory or operational management. Palliative Care experience a plus.
• Experience in multi-site home care programs a plus.
• Minimum 2 years of experience monitoring healthcare external and internal patient satisfaction, quality measures and outcome measures. Home Health and Palliative Care standard of care and quality measures.
• Ability to prioritize key trends and effectively communicate outcomes to diverse audiences.
• Facilitate continuous improvement. Lead performance discussion focusing on goals, interventions and timelines.
• Ability to lead improvement projects utilizing evidence-based framework (e.g. Rapid Cycle, Lean Six Sigma, Baldrige Framework and or PDSA)
• Competent in Microsoft Office Applications including Excel, Word, and Power Point to create reports.
• Dedication to improving customer experience and positive outcomes.
