Empath Health is seeking an Full-Time Private Duty Intake Coordinator to support our mission of compassionate, full-life care. This hybrid role blends administrative support for senior leaders and the Business Development Team with intake and enrollment coordination for participants and families.

What You’ll Do
- Coordinating the referral process by ensuring by ensuring a seamless review, response, and care coordination for patients.
- Reviews referrals to determine the appropriate care setting and communicate effectively with the care center, Business Development, and referral sources.
- Coordinates with other departments for insurance verification and obtains payer authorization as necessary.
- Serve as the first point of contact for new participants and referral sources by providing excellent customer service.
- Coordinate home visits, enrollment paperwork, and Medicaid processes.
- Provide excellent customer service to families, partners, and community agencies.
Position Requirements
- High school diploma or GED equivalent
- Licensure:
o Valid Florida LPN License - 1+ years of supervisory experience or demonstrated ability to manage a team; 2+ years experience in Home Health referral management or a related field.
- Knowledge of Medicare/Medicaid and community resources a plus.
- Clear understanding of medical terminology and basic ICD-10 coding.
- Strong skills in Microsoft Office, communication, and project management.
- Ability to manage confidential information with professionalism.
- Valid Florida driver’s license, insurance, and reliable transportation required.
- Flexibility for occasional evenings, weekends, and local travel.
