Job Description

  • Deliver skilled PT interventions across home, Day Center, and facility settings
  • Collaborate with the care team to personalize therapy plans
  • Manage therapy documentation and track participant progress
  • Coordinate use of durable medical equipment (DME)
  • Support restorative, maintenance, and rehab goals in a proactive care model

 Position Requirements

  • Bachelor’s in Physical Therapy required (Master’s preferred)
  • Florida PT license (or eligibility)
  • 1+ year of experience with geriatric or frail populations
  • CPR certification (AHA or Red Cross)
  • Strong communication, time management, and documentation skills
