Care Team Manager

The Care Team Manager:  

  • Effectively manages a team of clinical and support colleagues working in an in-patient environment, maintaining accountability for providing the highest quality of clinical care and psychosocial support for patients and families.
  • Provides positive leadership of a clinical team composed of a variety of disciplinary practices to ensure patient care efficiency, effectiveness and satisfaction. Promotes continued professional development.
  • Serves as clinical team expert and manages high risk and complex patients/family/staff situations, accessing available resources, maintaining effective communication and optimizing quality patient care.
  • Collaborates and communicates effectively with associate medical director to initiate appropriate level of care on admission and changes in level of care. Co-facilitates Interdisciplinary Group meeting.
  • Performs ongoing review of patient documentation to ensure the documentation is complete, accurate and timely and to verify that the electronic medical record system (EMR) is being utilized appropriately.
  • Maintains responsibility for bed management activity and monitoring inpatient length of stay. Monitors for staffing and census fluctuations and provides direct patient care as needed.
  • Ensures compliance with all accrediting agencies, State, Federal, and local government regulations; maintains a strong position against fraud and abuse.

Educational/Professional:

  1. Must possess current and active Florida licensure as RN; BSN preferred. CHPN a plus.
  2. Minimum of 2 years recent community-based hospice experience required.
  3. Recent acute level clinical experience required; GIP care experience preferred.
  4. Previous management experience required; healthcare environment preferred.
  5. An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
